Pressure-ulcer stages

The pressure-ulcer stage is determined using the European Pressure Ulcer Advisory Panel classification:

  • Stage I. Non-blanchable erythema of intact skin. Additional signs (especially on darker skin) — changes in skin colour, warmth, swelling, a hardened area.
  • Stage II. Partial-thickness skin loss affecting the epidermis and/or dermis; superficial, presenting as an abrasion or ulcer.
  • Stage III. Full-thickness skin loss with damage to or necrosis of subcutaneous tissue, possibly down to but not through the underlying fascia.
  • Stage IV. Extensive tissue damage or necrosis involving muscle, bone or other supporting structures.
